I CONFIRM I am able to attend the 2023 Youth Tour on June 16-22, 2023 (Oklahoma) or June 15-22, 2023 (Kansas). I understand that if I win the contest and forfeit the trip that I may be required to refund my sponsoring cooperative for all or some of the cost.
I hereby give permission for images of myself (or child), during Youth Tour application process through video, photo and digital camera, to be used solely for the purposes of the sponsoring cooperative, statewide and national electric cooperative associations promotional material and publications, and waive any rights of compensation or ownership thereto.
